New Patient Registration

We are currently not taking any new patients on to our registered list, we hope to be able to register new patients in the future.

We are unfortunately unable to register any new patients at this time. Our practice has been experiencing a significant increase in the number of registered patients over the last few years, and we do not have the space to accommodate enough staff to care for any additional patients in our current building.

The only exceptions that we are able to make are that we will still register newborn babies or adopted children of our current registered patients.

This does not affect the care of patients who are already registered with us, and they will continue to receive healthcare services as usual.

If you are not currently registered with a local GP practice, it is still important that you register in order to receive NHS services. There are a number of other GP practices in the local area, and you can find out about them and how to register with them by looking at https://www.nhs.uk/Service-Search/GP/LocationSearch/4

If you wish to register with the practice you may either call into the surgery to collect the relevant documents to take home and complete or alternatively print all of the below documents, complete the information required and bring to the surgery along with 2 forms of identification.

The New Patient Registration document includes a medical questionnaire which you will be required to complete as it can often take considerable time for us to receive your medical records.

  1. GMS1 Registration Form
  2. New Patient Registration Questionnaire
  3. Alcohol Audit Questionnaire
  4. Great Oakley Medical Centre Confidentiality Consent Form
  5. Carers Identification and Referral Form
  6. Family Questionnaire for Registration
  7. Data Matters-NHS
  8. Patient Preferred Contact

When you register we will ask you to have some basic health measurements undertaken by our automated machine in the waiting area, or if you feel more comfortable seeing a nurse to do this please make an appointment to do so.

If you are taking regular medication we ask you to bring a copy of your list of medications you take when you bring your registration documents in this will enable your repeat prescriptions to be processed.
